Job Summary
We are seeking a compassionate and skilled Occupational Therapist (OT) to join our Long-Term Care team. The Occupational Therapist will evaluate residents, develop individualized treatment plans, and provide therapeutic interventions aimed at improving residents' ability to perform activities of daily living (ADLs), enhance independence, and improve overall quality of life in a skilled nursing and long-term care setting.
Key Responsibilities

  • Evaluate residents' physical, cognitive, and functional abilities upon admission and as needed

  • Develop, implement, and update individualized occupational therapy treatment plans

  • Provide therapeutic interventions focused on ADLs, mobility, strength, coordination, balance, and safety

  • Educate residents, families, and caregivers on adaptive techniques, equipment, and home safety strategies

  • Recommend and train residents on the use of adaptive equipment and assistive devices

  • Collaborate with interdisciplinary teams including physicians, nurses, PTs, SLPs, and social services

  • Accurately document evaluations, progress notes, and discharge summaries in accordance with facility, state, and federal regulations

  • Participate in care plan meetings and contribute to resident goal setting

  • Ensure compliance with Medicare, Medicaid, and facility policies and procedures

  • Maintain a clean, safe, and organized therapy environment
Qualifications

  • Graduate of an accredited Occupational Therapy program

  • Current Occupational Therapist license in the state of practice

  • CPR/BLS certification (or willingness to obtain)

  • Experience in Long-Term Care or Skilled Nursing preferred (new graduates welcome)

  • Strong clinical, communication, and documentation skills

  • Ability to work independently and as part of a multidisciplinary team
Skills & Competencies

  • Knowledge of LTC regulations and documentation standards

  • Patient-centered care approach with strong empathy and professionalism

  • Time management and organizational skills

  • Ability to adapt treatment plans to meet diverse resident needs
Physical Requirements

  • Ability to lift, push, and pull up to 50 lbs with assistance

  • Ability to stand, walk, bend, and assist residents throughout the shift
