About St. Mary's Center:
St. Mary's Center is the only HIV Residential Facility on the Upper West Side of Manhattan. Since the Skilled Nursing Facility (SNF) opened in 1992, there have been tremendous improvements in the management of HIV, leading to increased life expectancy, an aging HIV-positive population, and the emergence of chronic medical conditions similar to those found in non-HIV populations.

St. Mary's Adult Day Health Care (ADHC) program was established to address the medical, psychosocial, and emotional needs of this expanding group of chronically infected clients. Our interdisciplinary team includes physicians trained in internal medicine and HIV care, nurses, therapists, and social workers. Together, we provide comprehensive, compassionate care to support clients in achieving stability, independence, and quality of life.

Brief Description
The Licensed Master Social Worker (LMSW) provides psychosocial assessments, counseling, and case management services to clients in the HIV Residential and Adult Day Health Care programs. The LMSW collaborates with medical and nursing staff to develop and implement individualized care plans that address clients' medical, emotional, and social needs. This role supports St. Mary's mission to provide the highest quality, person-centered care while promoting client empowerment, wellness, and community integration.

Job Duties / Responsibilities

  • Conducts comprehensive psychosocial assessments and develops individualized service plans in collaboration with the interdisciplinary team.

  • Provides individual and group counseling focused on coping skills, adjustment to illness, medication adherence, and mental health support.

  • Offers case management, including coordination of medical, mental health, housing, and social services for residents and ADHC clients.

  • Advocates for clients' rights and access to community resources, entitlements, and benefits.

  • Participates in interdisciplinary care meetings, contributing to care planning and discharge planning processes.

  • Maintains timely, accurate documentation in accordance with agency and regulatory standards.

  • Provides crisis intervention and supports clients in managing behavioral or emotional challenges.

  • Assists clients in setting and achieving personal recovery and wellness goals.

  • Educates residents and families about available services, treatment options, and community supports.

  • Participates in quality improvement initiatives and program development activities.

  • Supports the creation of a safe, inclusive, and trauma-informed care environment.

Qualifications

  • Licensed Master Social Worker (LMSW) in New York State (required).

  • Experience working with individuals living with HIV/AIDS, substance use disorders, and/or chronic medical conditions preferred.

  • Strong knowledge of community resources, entitlement programs, and psychosocial interventions.

  • Excellent communication, organization, and teamwork skills.

  • Commitment to cultural humility and person-centered care.
